Enrollment for SY 2017-2018

Welcome to Bridges Public Charter School (Bridges PCS) enrollment registration process. There are 2 main steps to complete  the enrollment of the student at Bridges PCS. First step is to complete the online forms. The second step is to print, complete, and submit paper forms and provide required documentation to the school. If you need assistance to complete the online forms, you can do this by setting up an appointment with Ms. Katherine or Ms. Mary between the time frame of 9:00 am - 2:30 pm. You may submit documents between 8:30 am -2:30 pm (no appointment necessary for document submission). 

STEP 1 - Online Forms

1. You should have received an email that includes a 15 character snapcode. If you have NOT received this email, please get in contact with Ms. Mary or Ms. Katherine at 202-545-0515 so that you can receive a print out, you will not be able to complete the online forms without this code. 

2. After you have received you snapcode, please begin filling out the online forms by clicking on this link: https://secure.infosnap.com/family/gosnap.aspx?action=10756&culture=en

STEP 2 - Paper Forms and Document Guidance

To confirm the registration of the student at Bridges PCS please print, complete, and submit the following paper forms and provide the required documentation to the school.   2. DC Residency Verification Form & Supporting Documents (Click here for List of Acceptable Supporting Documentation)

  3. Proof of Legal Guardian’s Identity (Any state issued Photograph ID).

  4. For new students to Bridges PCS please submit a copy of the student’s Birth Certificate if you have not uploaded it directly as part of the online enrollment registration form.

  5. Please fill out this form if you would like to participate in the Free and Reduced School Lunch Program - Click Here

    • Students will qualify for free and reduced meals depending on the income eligibility guidance provided by USDA - https://www.fns.usda.gov/school-meals/income-eligibility-guidelines. If the student does not qualify for Free and Reduced meals there will be a monthly invoice for lunch. 

    • If you do not plan to participate in the school lunch program please make sure to send a lunch daily.

  6. If the student is new to Bridges and has an IEP (Individualized Education Program) - if copies of the student’s IEP and related assessments from the prior school is available with you, please bring it with you.

  7. If the student is new to Bridges and enrolling from another school please submit the records release form and a copy of the final report card if available. 

  8. If the student is enrolling in Bridges after the student started attending another school in SY17-18 please provide a Transfer / Withdrawal Form from previous school. (Only applicable after SY17-18 school year has started.)

Health Forms

Please find below the official health related forms. The forms have to be printed and filled out by the student's physician unless mentioned otherwise. The forms will also need the signature of the parent or legal guardian as per the instructions on each form. All of the forms have to be completed and submitted to the school. Every student should have an updated health (which includes immunization records) and dental exam to begin the school year. If your child needs medications at the school please make sure to complete the Medication Authorization Form. (On the first day of school please provide the school with an unopened prescription.)

  1. Official Emergency Medical Treatment Authorization Form. This form has to only be completed by the primary legal guardian / parent. - Click Here

  3. District of Columbia Oral Health Assessment Form. This form has to be completed by the student's dentist - Click Here

  4. Official Medication Authorization Form. If applicable, please provide this completed form to the school - Click Here

  7. If you have not uploaded a copy of the Health Insurance Policy card covering the student in the online registration form please bring a copy of it with you.
