Every child should visit their primary care provider annually and their primary dental provider every six months to receive preventive care. According to DC laws and regulations, children attending school or daycare in DC must submit a completed Universal Health Certificate and Oral Health Assessment Form each year to ensure they are receiving necessary health care.
The Universal Health Certificate and Oral Health Assessment must be provided to front office staff. Parents and guardians are required to provide the school nurse with any authorized medication along with the proper medication form. This ensures that students receive the necessary medication in a safe and timely manner while at school.
