Health Services

Every child should visit their primary care provider annually and their primary dental provider every six months to receive preventive care. According to DC laws and regulations, children attending school or daycare in DC must submit a completed Universal Health Certificate and Oral Health Assessment Form each year to ensure they are receiving necessary health care.

The Universal Health Certificate and Oral Health Assessment must be provided to front office staff. Parents and guardians are required to provide the school nurse with any authorized medication along with the proper medication form. This ensures that students receive the necessary medication in a safe and timely manner while at school.

    Exemption Process

    To request a religious exemption to the school vaccination requirement for your student or child:

    Please submit the name of each child, their date of birth, and the school or childcare facility where they are or will be enrolled in an email to doh.immunization@dc.gov. We will respond to get further information from you.\ OR

    Come in person Monday–Friday, 9:00 am to 4:00 pm to:\ DC Health\ Immunization Division, 5th floor\ Community Health Administration\ 2201 Shannon Place SE,\ Washington, DC 20020

    Annual Submission of Immunization Exemptions and Ongoing Healthcare Requirements

    Both medical and religious immunization exemptions must be submitted annually to ensure compliance with DC regulations. However, it is important to note that these exemptions do not relieve students from the requirement to receive their annual care from their primary care provider. All students must continue to receive their routine medical check-ups, regardless of their immunization status.
